CHW Program Database

The Pacific and Southwest Regional Health Equity Council (Pacific and Southwest RHEC) works on health equity issues across an expansive region that spans from Arizona in the east to the Republic of Palau in the west. The Pacific and Southwest RHEC’s Community Health Workers (CHW) Committee developed this regional CHW program database to include existing CHW programs, populations served, services offered, language capacity, CHW collaborators, funding sources, and CHW websites. The purpose is to begin to create a regional map of CHW work and stakeholders in the region.

The Pacific and Southwest RHEC would like to invite you to use the database to engage CHWs, CHW trainers and CHW employers to support community empowerment, health and vitality. We also invite you to email us with updates to the database, and to share best and promising practices for integrating CHWs into care teams. Ultimately, your feedback will help promote regional advocacy efforts to expand and sustain CHW programs and workforce, and help to build innovative partnerships between private and public sectors.

Our hope is to enhance prevention and access to health care, so we would appreciate your participation and feedback. Specifically:
  1. Are there organizations or programs that need to be added?
  2. What experiences have you had with the organizations or programs that are listed that would contribute to a promising/best practice?
  3. Are there organizations or programs listed that you would recommend as workshop or webinar presenters?
  4. General updates to the database?
